That's rather an ironic quote, as Ayn Rand spent her professional life ranting about the "evils" of the State providing care and support for its most vulnerable citizens, only for reality to bite the bug-eyed benzedrine fiend on her bum when after decades of heavy smoking she got cancer and fell on hard times herself, at which point she ponced off welfare for six years before her death.

Variolation worked but carried a significant risk of severe disease or even death in the recipient. This risk was reduced dramatically by substituting smallpox material by fluid from a cowpox lesion.
